3. Bureau of Motor Vehicle Driving Abstract. You may secure a copy of each of your driving records by completing the BMV 1173 form with $2.00 per request and mailing it to:

This should be returned to you – not Adoption Circle. If you have not resided in Ohio for the past 5 years, you must secure driving abstracts from previous states of residence.

4. JFS O1653 "Medical Statement for Adoptive Applicant and All Household Members". Your physician MUST complete and sign and date the entire 2nd page of this form, including professional opinion section.

5. 4 References (minimum 3 non-relative). References must be from the same individuals listed on the JFS O1691. Adoptive Applicant Reference Forms are enclosed. You complete SECTION I with your name(s). The person providing the reference completes SECTION II AND III and then returns the form to Adoption Circle.
